Based on physical activity and behavioural change, healthcare professionals can refer patients to the Respiratory Programme via an easy to use online portal, or patients can also self-refer if they feel they would benefit.
Once the referral is received, the client will initially be contacted by the Live Life Well Aberdeenshire team to discuss options and offer support, advice and encouragement to begin.
The client's details are then passed to trained, specialist instructors who will continue to provide support tailored to the needs of the individual.
During the Covid-19 pandemic, the team is concentrating on improving physical activity using online classes, led Health Walks, One to One training (dependent on circumstances) and supported self-management, where attending an online class or Health Walk isn't possible.
The service also includes follow up support, where on completion of a programme telephone contact will be maintained to help ensure people are able to maintain physical activity.
Live Life Well Health and Wellbeing instructors have specialist qualifications in areas including falls prevention, cardiac illness and cancer.
Other instructors are trained to deliver classes for those with long term health conditions, including Chronic Obstructive Pulmonary Disease (COPD).
Health Walk leaders are trained by Paths for All and can also deliver strength and balance activities.
